Registration Requirements

Chris Smith Basketball Camp will run from August 12-16, 2013. A non-refundable deposit of $50 is required with all applications. All balances must be paid in full by the morning of August 12, 2013. An updated physical, and a medication authorization form if child requires medication, inhalers, and epi-pens are required upon registration.
